Herpes simplex virus, Varicella zoster virus
Dosage form
250 mg powder/vial, 500 mg powder/vial
Dose
Adult :
Severe Herpes Simplex Infections: 5mg/kg/dose every 8 hours.
Varicella zoster infections: 5 to 10mg/kg/dose every 8 hours.
Herpes simplex encephalitis: 10mg/kg/dose every 8 hours.
Child :
< 12 years of age: 250mg per m2 /dose every 8 hours.
>= 12 years of age: 5mg/kg/dose every 8 hours.
Herpes simplex encephalitis: 5 to 10mg/kg/dose every 8 hours

Adverse effect
Neurotoxicity (delirium, tremor, lethargy, stupor, coma, nausea, vomiting, seizures), renal dysfunction, elevated serum creatinine (rapid intravenous injection), injection site (phlebitis, erythema), rash, urticaria, nausea, vomiting, abdominal pain, haematuria
Precaution
ระวัง Hypersensitivity ก่อนให้ยา.ห้ามให้ rapid or bolus intravenous injections. ระวัง extravasation และไม่ให้ผ่านทาง subcutaneous หรือ intramuscular injections. ถ้าความเข้มข้นเกิน 7 mg/ml อาจให้ช้าๆผ่านทาง central venous, Monitor full blood count (white blood cells with differential and platelets) and serum creatinine regularly during therapy
Contraindication
ถ้าเกิด phlebitis อาจพิจารณาให้ peripheral injection site และ/หรือลดความเข้มข้นของยา หรือให้ยาทาง central venous
